シミュレーション教育履修が必須化 ― 米国麻酔専門医資格更新制度

そのキャリアを通じて、麻酔専門医の専門性並びに最新知識を担保すべく定められたのがMOCA（Maintenance of Certificate in Anesthesiology）。

2011年より米国で制度化されました。これにより 米国ではシミュレーション教育受講が麻酔専門医 資格更新の条件となりました。受講は任意では ありません。

このシミュレーション教育受講履歴がない場合、麻酔専門医としての資格が更新出来ません。今回は、この制度概要に付き御紹介します。

 

■ MOCA　（Maintenance of Certificate in Anesthesiology）

MOCAは単なる専門医資格の更新手続きではなく、このプログラムを通じ麻酔専門医の生涯にわたる質と患者の安全性向上を図ることが目的とされています。1サイクルが10年のスパンとなっており、その中で麻酔専門医に必要とされる６つの分野での知識・技術向上の機会が提供されます。具体的には、1．医学知識、2．患者ケア、3．実務レベルの学習と改善、4．専門性、5．人間関係を含めたコミュニケーション・スキル、6．システム・ベースのプラクティスがそれです。

これらの習得・向上、及びに麻酔専門医資格更新手続き自体の為にMOCAプログラムは下記の4つのパートから構成されています。

Part 2.	Lifelong Learning and self-Assessment
Part 3.	Cognitive Examination
Part 4.	Practice Performance Assessment and Improve

2000年以降の麻酔専門医資格取得者は、自動的・そして強制的にこのシステムに組み込まれます。2000年以前の資格取得者に就いては任意とされておりますが、プログラムへの参加が奨励されています。

有資格者が更新の為に必要とされるコスト総額（テスト・研修参加ほか、10年間1サイクルをこなす為の）は約＄10,000（80円換算で80万円）かかるとの事です。

 

■ 要件とされるシミュレーション教育とはどのようなものなのか？

上記したMOCAの4つのパートの"Part 4"に位置するのが、シュミュレーション教育履修です。ABA（American Board of Anesthesiology）はシミュレーション教育を革新的なアプローチとして捉え、観察・改善手法の効果的手段としています。あくまでも履修することが条件で、受講者の技量・知識を評価するテストではありません。履修を通じ麻酔専門医の技術向上・エラー削減・エラー発生時の対応技術ほかを習得して貰う為のものです。

基本的には、受講者4-6名・インストラクター2-3名／コース。

6-8時間の履修が条件となります。また受講料は$1,500／コース（80円換算約12万円）。

施設の出身者・関係者による受講、並びにグループ受講には割引を設けたり、履修後30日後・6か月後のフォローアップ研修を設けたりしている施設も多い様です。

教育内容・時間に就いては、ASAのガイドラインに沿うことが求められますが、施設により若干の違いがあるようです。

但し、高機能患者シミュレータで臨床症例を再現し、臨床対応・リーダーシップ・危機管理等を学習する内容は必ずアジェンダに含まれています。認定サイトであるThe Milton S. Hershey Medical Centerのアジェンダを下記御紹介致します。内容から、概ねどのようなシミュレーション・プログラムなのかお分かりいただけるかと思います。

年間5回程度　　AM8:00-PM4:00　　受講料$1500（80円換算12万円　朝食・昼食付き）
同校出身受講者には25%までの受講料割引あり。

（内容） Difficult Airway, recognition and treatment of ischemia and cardiac arrest, And have the opportunity to manage rare and critical pulmonary and cardiovascular events 等

■デューク大学　(Duke University Human Simulation and Patient Safety Center)

年間5回程度　AM7:30-5:00　受講料$1550　（朝食・昼食・コーヒー代込）

受講者が4名に達しなかった場合はコース順延。シナリオ終了ごとにデブリーフィング実施。

（内容） Simulation training requirement for part IV(Practice Performance Assessment & Improvement) of “MOCA”. Course objective: 1. Provide management of hemodynamic and respiratory critical events in anesthesia 2. Practice up-to-date techniques in a safe learning environment. 3. Reflect on optimal teamwork, leadership and communication skills required in crisis management.

■カリフォルニア大学LA校　（UCLA　Department of Anesthesiology）

受講料$1500　（グループ参加及びにUCLA関係者の場合は$1300）。受講者は4-6名、インストラクター2-3名。

受講者が5名に達しなかった場合は順延。

 

■ 要件とされるシミュレーション教育とはどのようなものなのか？

・ASAの指定サイトのみで履修が可能です。

・指定サイトとなる為には施設機材・経験・実績ほか幾つかの要件があります。

・現在米国で27の施設が認定施設となっていますが、今後認定施設は増えて行く予定との事です。
